CON Simulation Center Process for Support & Space Requests

Step 1

Fill in the Support and Space Request form

Requestor/faculty will receive an email, stating “Request Under Review” and a tentative event is created in their calendar.

Kindly provide time for the SIM team to review your request.

Step 2

If the request is approved, the requestor will receive a link to complete a Set Up Request Form & Standardized Patient Request Form (SP form optional & only for CON Faculty)

Note: Requestor needs to enter the correct activity id referenced in the email when filling out these two forms for all the requests to match up.

Step 3

If the request is rejected, the requestor will be notified by email but can contact the Simulation Team at nursingsim@usf.edu for an alternative date and time.

Step 4

The SIM team will review all requests. If everything is good to go, the requestor will receive an email stating “Activity Approved”. This marks the end of the process of requesting space & support in the simulation lab.
