Application Procedures
The Department of Orthopaedics and Sports Medicine at the University of South Florida will provide two ACGME-accredited orthopaedic sports medicine fellowship positions for the 2011-2012 academic year. The deadline for application is November 23, 2009. The Sports Medicine Match is administered by the San Francisco Match program. For further information regarding the application process, please go to their website (http://www.sfmatch.org/).
Most of the important information will be included in the San Francisco Match application. In addition, candidates should provide the following information directly to our program, via Ann Joyce, prior to the November 23, 2009 deadline:
- Copy of doctoral diploma (M.D. or D.O. degree)
- Copy of DEA license and current state licenses
- Three (3) letters of recommendation (if not transmitted via the San Francisco Match)
- Confirmation that the candidate is currently eligible for a Florida state medical license
Applications should be received 19 months prior to the start of their fellowship and interviews conducted in January or February of the ensuing year. A great deal of emphasis will be placed on the interviews. Each applicant will be interviewed by the full time faculty and have general sessions with adjunct faculty in primary care sports medicine, athletic training and physical therapy. Applicants will then be ranked and then submitted to the NRMP match, which will be in compliance with the
recruitment and selection of the USF COM policies.

Foreign medical school graduates: As a general policy, only trainees who are U.S. citizens or permanent residents at the time of application are considered for clinical ACGME accredited positions in our fellowship training program. However, in highly unusual situations, a J-1 visa holder with an outstanding prior record of research or other outstanding academic credentials may be considered. Please contact Ann Joyce, ajoyce@health.usf.edu for additional information. ALL DOCUMENTS IN A FOREIGN LANGUAGE MUST BE ACCOMPANIED BY A CERTIFIED TRANSLATION IN ENGLISH.
