Collaborative Agreements
If you are a faculty member wishing to initiate a Collaborative Agreement between USF and another institution, please follow the instructions below.
1. Download the USF Health International Review Form for Linkages and the General Agreement for Collaboration from the College of Medicine International Affairs Website.
2. Obtain the names, titles and contact information for the appropriate persons of authority at the partnering institution, as well as a complete name and address for the institution.
3. Insert institution name and appropriate authority names and titles in both the Review Form and the General Agreement.
4. Make two originals of the Collaborative Agreement (after all signatures are obtained, the USF Office of International Affairs will need one original and the partnering institution will need to keep an original). Note: If the partnership is in a country that speaks a language other than English, there will need to be two originals in English and two originals in that country’s language.
5. The USF Faculty member initiating the partnership should sign the Review Form in the “Submitted for review by” section and forward the Review Form with the two originals of the General Agreement to:

6. The COM International Affairs office will obtain remaining internal signatures on both the review form and the two originals of the agreement and will notify you when they are complete.
7. The two Collaborative Agreements will then be ready to forward or be taken to the partnering institution.
After signatures from the partnering institution have been obtained, one original of the Collaborative Agreement should be returned to COM International Affairs at the above address. COM International Affairs will forward the agreement to the International Affairs Center. Note: Please remember to make a photocopy of the agreement for your records.
Participation in our programs is restricted to institutions that have initiated a Collaborative Agreement with the USF College of Medicine. Currently, the following universities:
- Yeungnam Medical Center, Yeungnam University, Daegu, South Korea
- Universidad San Francisco de Quito, Ecuador
- Universiteit Utrecht, Randstad, Netherlands
- Albert-Ludwigs-Universitat, Freiburg im Breisgau, Germany
- Faculty of Medicine, Srinakharinwirot University, Nakorn Nayok, Thailand
